The above-numbered RFP is amended as set forth below. Respondents must acknowledge receipt of this addendum prior to the hour and date specified for receipt of quotes, by signing and completing the last page of this addendum. Failure to acknowledged receipt will result in disqualification of bid. In order to ensure that all firms are given an equal opportunity to submit a competitive response, the following are clarifications concerning this solicitation. Juanita Gardine School Roofing Proj. - Addendum #1 Lead Base Paint Abatement Scope and Bid Schedule: For IFB #IFBO00S5DOEC16(C) Item #: Work Description Inspect and professionally secure paint samples from soffit areas as detailed under Task #6 & #8 for uilding #5 & #6 soffit areas to determine the possiblity of the presence of Lead Base Paint contaminates: Analytical Lab Testing & Results of properly secured Paint Samples by an EPA approved Laboratory: Part A Subtotal Lump-Sum: Bl Securing all required Permits to perform any required Lead Paint Abatement work efforts: B2 Removal & Abatement of all Lead Base Paint Contaminated Materials by EPA licensed & certified jabatement workers: B3 EPA approved Packaging & Transportation of Lead Base Paint containing Materials to an EPA pproved disposal site on the US Mainland: B4 {Certification Documentaion of Lead Base Paint Abatement Completion: Part B Subtotal Lump-Sum: In addition to the standard V.I. Governmental compulsory General Liability Insurance, |Lead Base Paint Liability, as well as, Professional Liability Insurance is required in the amount of 1,000,000.00 for Contractor selected Hazardous Waste Company for the above Abatement Work #B1-B4. AN2 __|Each Bidder must attend the Mandatory Site Walkthrough to submit a 'Responsive' Bid. AN3 __|AIl work shall be inspected and approved by DOE, DPW and DPNR prior o acceptance by DOE.
