Box 302702, St. Thomas, Virgin Islands 00803 hereinafter called the “CONTRACTOR”, WITNESSETH: WHEREAS, the Government is in need of a contractor to provide exterminating services to the Department of Public Works in the St. Thomas/ St. John District; and WHEREAS, the Contractor represents that it is willing and capable of providing the services tn an expeditious manner and in accordance with the specifications cited in Addendum I, II, Il, and IV all attached hereto and made a part hereof; it is mutually agreed between the Parties as. follows: SECTION 1. That for and in consideration of the price or prices and. agreement-in Addendurn II (Compensation) hereto attached and made a part of this Contract, the said Contractor agrees to provide. exterminating services to the Department of Public Works in the St. Thomas/ St. John District with the provisions, of Scope of Work (Addendum. I), General Provisions (Addendum II), and Termination of Contracts (Addendum IV), any Supplemental Provisions and Specifications and the Purchase Order, including any change thereof, are all part and parcel of this Contract and are by this. reference, incorporated in this Contract-as fully and effectively as if set forth in detail herein. SECTION 2. The Government, for and in. consideration of the full and true performance of the work ‘by the Contractor, agrees to pay the price or prices set forth in the attached in the line items as indicated in Compensation (Addendum ID), in lawful money of the United States,.and the payment shall be made at the time and in the manner set forth in the Compensation the General Provisions, S006DEWT21 (OEST) Page i Initial Ay ADDENDUM I Scope of Work All pest control work shall be performed in a safe manner and accordance with the Integrated Pest Management (1PM) procedures. Application procedure for the treatment of roaches, ants and silverfish will be. applied by spraying on and around infested areas and areas that the pests will become in direct contact with. Ant and roach gels can also be used to eliminate pest. Name and nomenclature of products that are used for roaches, ants, and silverfish are Advion or Alpine (gel bait), Suspend SC, Demand CS (emulsifiable concentrate), or Cypher (Wettable Powder). Application procedure for treatment of rodents: Rodent bait: stations will be placed on. the interior and around the exterior of the building as needed. Name.and nomenclature of products that will be used for rodents: Final Blox Oliver Exterminating of St. Thomas, Inc Material Safety Data Sheets (MSDS) are available forall the chemicals that we administer for the control and prevention of your pest problems.
