Virgin Islands, hereinafter called the "GOVERNMENT™. and Franklin Marte d/b/a Real Tech Exterminating. whose address is P.O. WITNESSETH: For, and in consideration of the acceptance of the Contractor's proposal. under Invitation for Bids No. IFBO30DHSC20 (S) opened on April 24. 2020 and the award of this contract to the Contractor. notification hereof having been made to the Contractor on July 20, 2020 and in further consideration of the covenants and agreements of the Parties herein contained, to be well and truly observed and faithfully kept by them. and each of them. it is mutually agreed between the Parties as follows: SECTION |. That for and in consideration of the price or prices and agreement in this proposal hereto attached and made a part of this Contract. the said Contractor agrees to furnish and deliver any or all of the commodities described in the said Invitation for Bids No. IFBO30DHSC20 (S) and the Scope of Work (Addendum J) at the price or prices stated therein and in strict accordance with the conditions of said accepted proposal. The Advertisement. Invitation for Bids. General Provisions (Addendum II). fermination of Contracts (Addendum IV). any Supplemental Provisions and Specifications and the Purchase Order. including any change thereof. are all part and parcel of this Contract and are by this reference. incorporated in this Contract as fully and effectively as if set forth in detail herein. SECTION 2, The Government. for and in consideration of the full and true performance of the work by the Contractor. agrees to pay the price or prices set forth in the attached Invitation for Bids and the line items as indicated in Compensation (Addendum I1), in lawful money of the United States. and the pa\ ment shall be made at the time and in the manner set forth in the Invitation for Bids and the General Provisions. SECTION 3. This Contract shall commence on upon execution of the Commissioner of Property and Procurement and shall terminate one year thereafter. unless mutually extended or terminated by the parties. The services under this contract shall be for a period of one (1) year with a renewal option for a period of one (1) year No alterations or variations of the terms of the proposal shall be valid or binding upon the Government unless made in writing and approved by the Government. SECTION 4. This Contract will remain in force for the full period specified and services of termination shall be satisfactorily delivered and accepted and or until all terms and conditions have been met. unless: ADDENDUM I Scope of Work Franklin Marte d’b-a Real Tech Exterminating will provide exterminating services for the Department of Human Services at: {. The Youth Rehabilitation Center- Location: 6179 Anna's Hope. Christiansted. V1 2. Anna's Hope-Group Home Location: 6179 Anna's Hope. Christiansted. VI The Youth Rehabilitation Center (YRC) includes: 1. The Main building consists of 46 raams with 3 bathrooms 2. Annex consists of 38 reonis with J bathrooms 3, Education buildings (2) consist of 10 reams with.3. bathrooms 4. Kitchen‘Cateteria consists of 4 rooms Franklin Marte db’a. Real Tech Exterminating will provide exterminating services at the Youth Rehabilitation Center (YRC): . e Once a month for the regular buildings. e Twice a month for the Kitchen:Caleteria ¢ Preferred Service Day: Saturday Monthly Task Assignment: | e External -provide and monitor bait houses on esterior of building for rats and other pest control which includes bees. » Internal - spray offices. bedrooms. bathrooms. main lobbies. lounge, kitcheneites, main work floors, cafeteria‘kitchen. conference room. and along all internal perimeters. of the buildings: The Anna’s Hope: Group Home inctudes: 1, Eight (8) offices 2. Five (3) bedrooms 3, Four (4) bedrooms +. Two (2) slorage rooms 4. Kitehendining area Franklin Matte-d/b‘a Real Tech Exterminating will provide exterminating services at the Anna’s Hope Group Hame: ¢ Once a menth for all-areas at the Anna's Hope Group Home. e Preferred Service-Day; Any weekday. Monday-Friday Monthly Task Assignment: * External -provide and monitor bait houses on exterior of building for rats and other pest control which tietudes bees. ¢ Internal - spray offices. bedrooms. bathrooms. lounge. kitchen. main. work floors, cafeteria/kitchen. and along all internal perimeters of the buildings.
