There are Federally and Territorially Listed endangered spe- cies confirmed to be on the Island, such as the Federally Listed species: Green Sea Turtle, Hawksbill Turtle, and Brown Pelican; and Territorially Listed species: White- cheeked Pintails and Malpighia Woodburyana. There are other additional Federally Listed species that could potentially be on the island, but have not been observed (such as the Virgin Island Tree Boa, and St. Thomas Prickly Ash). It is located in the St. Thomas Bay between the East and West Gregerie Channels, South of the City of Charlotte Amalie, St. Thomas. Two distinct phases of military use occurred on Water Island. The first was the establishment of a coastal defense facility during World War II built to defend the Roosevelt Roads Naval facility on the island of Puerto Rico. The second phase was testing of chemical warfare materiels. The U.S. Government acquired Water Island in 1944 to serve as part of the coastal defense system for the U. S. Virgin Islands during World War II. The main fortification, Fort Segarra, located at the southern tip of the island, was constructed to protect the entrance to St. Thomas harbor. It consisted of two large gun em- placements, watchtowers, underground bunkers, and other mili- tary facilities. World War II ended before the two large guns could be installed. Two ammunitions storage igloos were built at Carolina Point. Other U.S. military installations existed on the main island of St. Thomas, including the U.S. Navy Submarine Base at Crown Bay and the U.S. Marine Corps Base at Bourne Field, now Cyril E. King Airport and the University of the Virgin Islands. From May 1948 to September 1950, the U.S. Army Chemical Corps occupied Water Island. Tests were conducted to evaluate various chemicals and their storage containers for use in tropical climates or jungle warfare missions. In 1952, the U.S. Department of Defense transferred Water Is- land to the U.S. Department of the Interior. The transfer of Water Island from the U.S. Department of the Interior to the U.S. Virgin Islands began in 1992 and was completed in 2005. Demolition explosives used for training purposes have been found on Water Island. Small arms ammunition have also been found and may still be present at the Former Fort Segarra. In- vestigations by the U.S. Army Corps of Engineers concluded that it is unlikely that chemical warfare materiel remain on Water Island. DELIVERY ORDER 0013 REV. 0 4/22/2009 FINAL CHAPTER 1 INTRODUCTION 1.1 PROJECT AUTHORIZATION 1.1.1 Parsons Infrastructure & Technology Group, Inc. (Parsons) served as the prime contractor to the U.S. Army Engineering and Support Center, Huntsville (USAESCH) under Contract W912DY-04-D-0005, Delivery Order 0013. This delivery order was established to conduct a Removal Action (RA) to remove, dispose, and document removal of potential 3X scrap, and also to design and install two informational signs at the former Ft. Segarra, Water Island, U.S. Virgin Islands. This project falls under the Defense Environmental Restoration Program for Formerly Used Defense Sites (DERP/FUDS). The FUDS project number for former Ft. Segarra is I02VI097701. 1.1.2 This Report covers the RA at Former Ft. Segarra identified in the Performance Work Statement (PWS) dated August 29, 2006 along with subsequent modifications. As specified in the delivery order, this report is prepared to summarize the work performed during the RA and presents an accounting of the recovered items. This report is prepared in accordance with the DID MR-030, as required by the PWS (Appendix A). This document describes the objectives, tasks performed, and documents generated as part of the RA at the former Ft. Segarra. 1.1.3 The work adhered to the Defense Environmental Restoration Program (DERP) for Formerly Used Defense Sites (FUDS) and the work was performed in accordance with the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) Section 104, and the National Contingency Plan (NCP), Sections 300.120(d) and 300.400(e). Additionally, the work under this task order was conducted in full compliance with Department of Defense (DoD), Department of Army, USACE, state and local requirements regarding personnel, equipment, and procedures. 1.1.4 Tasks for this project were awarded as Firm Fixed Price (FFP) tasks and Time and Materials (T&M) tasks. Details regarding the T&M costs incurred to perform the RA are included in Chapter 5, but FFP costs are not required in this report, per DID MR-030. 1.2 SITE HISTORY AND REASONS FOR REMOVAL ACTION 1.2.1 The U.S. government acquired Water Island (Figure 1.1) in 1944 for the Department of Defense (DoD) to establish a coastal defense installation. Fort Segarra was subsequently built at the southern tip of the island and consisted of World War II-era gun emplacements, barracks, watchtowers, underground bunkers, and other military facilities. In addition, docks, roads, water, sewage, and power systems were constructed. 1-1 l:\HUNT-MRS PROGRAM\PROJECTS\DOI3. At the end of World War II, construction on Fort Segarra halted and it was declared excess in 1946. 1.2.2 From May 1948 to September 1950, the U.S. Army Chemical Corps used Water Island for the testing and storage of chemical munitions as part of the Tropical Test Program. Test areas were designated on the western end of St. Thomas Island and in several areas on Water Island. The purpose of the tests was to evaluate the performance of chemical warfare materiel (CWM) in a tropical environment. The Tropical Test Program ended in 1950 and the operations were moved to Dugway Proving Grounds in Utah. 1.2.3 Historical records indicate that 57 CWM tests were planned for the Tropical Test Program in the U.S. Virgin Islands. However, only nine of these tests were actually conducted. As shown in Table 1.1, two of the nine tests were conducted on St. Thomas Island and seven were conducted on Water Island (U.S. Army Chemical Materiel Destruction Agency, 1993). 1.2.4 The disposition of chemical munitions at the end of the Tropical Test Program was detailed in a 1950 Army memorandum. Items were either dumped at sea at the Naval Ammunition Dumping Ground in the Caribbean Sea south of Vieques Island, or shipped to the Army Chemical Center, Maryland; the Midwest Chemical Depot, Arkansas; or the Dugway Proving Ground, Utah. According to the ASR, the only items not accounted for were four 500-pound M78 bombs that were taken out of the test inventory because the cyanogen chloride (CK) filler had solidified. Scrap from statically tested bombs may have been disposed of on-site (USACE, 2001). 1.2.5 Following the end of the Tropical Test Program in 1950, the Army granted a permit to the Department of the Interior (DOI) for use of the island and in 1952, Water Island was transferred to the DOI. The DOI subsequently leased the island to a private party (master lessee) for two 20-year terms for development as a resort area. This included the construction of a resort hotel on Rainbow Point, north of Flamingo Bay, beginning in the 1960s. The original master lessee, Water Island Incorporated, sold their rights in the lease to the Water Isle Hotel and Beach Club. The hotel is variably referred to in historical documents as the Water Isle Hotel and the Sea Cliff Resort Hotel. This lease expired in 1992. In December 1996, title was transferred to the U.S. Virgin Islands government for certain parcels of Water Island that included the catchment basin site and the hotel complex site and associated buildings. 1.2.6 Over the term of the lease, the master lessee granted numerous subleases. Many of the sublessees have constructed homes on their parcels. Many of the barracks, administration, and support buildings constructed as part of Fort Segarra have been converted to private homes. Some of these parcels have been transferred to the sublessees. 1.2.7 Hurricane Hugo in 1989 and Hurricane Marilyn in 1995 caused extensive damage to many of the structures on Water Island. Debris generated because of the hurricane damage was largely deposited in the southern portion of the island near Test Area 5. This includes a large amount of concrete debris generated during the demolition of the hotel in 1998. 1.3 PREVIOUS INVESTIGATIONS Previous investigations have included record searches, interviews, surface assessments, and an Engineering Evaluation and Cost Analysis (EE/CA). A summary of these investigations is provided below. 1.3.1. 1966 Ordnance Discovery at Flamingo Bay Landfill Area 1.3.1.1. In 1966, a Naval Ordnance Disposal Detachment at Roosevelt Roads, Puerto Rico, responded to an unearthing of metal objects that appeared to be bombs. The Navy report noted "several" bombs had been unearthed during a dredging operation at the Flamingo Bay Landfill Area, that they believed them to be Army M70 and M78 chemical bombs, that all but one had been vented, and that they blew the unvented bomb without noticeable release of any chemical (USACE, 2002). 1.3.1.2. Eyewitnesses described two bombs as 18 to 24 inches in diameter, both the same size, except that one had fins making it 8 to 12 inches longer. The bomb with fins was about five feet long. The eyewitness accounts did not say that the Navy had blown the items in place, but that they had taken the bombs with them and advised that no more excavation be performed at the site. 1.3.2. 1991 Archives Search Report and Site Visit In 1991, the USAESCH contracted Ebasco Services, Inc., to conduct an archives . search and site visit of the former Fort Segarra. This report and site visit resulted in the installation of a chain link fence around the perimeter of the Flamingo Bay Landfill. The 1991 ASR outlined the site history, site description, real estate ownership information, f results of a visual site inspection and interviews, and evaluation of potential ordnance contamination based on site information and archives search. This study summarized the background information, recorded disposition of CWM, and potential strategies for remediating potentially buried CWM. The report also identified regulatory and technical issues associated with efforts to clean up potentially buried CWM at the former Fort Segarra. 1.3.4. 1994 Management Technology Associates Site Characterization 1.3.4.1. In March 1993, Management Technology Associates, Inc. (MTA) was awarded a contract to perform a non-intrusive site characterization and debris removal operations at the former Fort Segarra. All field operations were completed by June 9, 1994. 1.3.4.2. MTA conducted a surface investigation of Test Areas 4, 5, 6, 8, Tamarind Bay, and the Flamingo Bay Landfill Area. Tamarind Bay is a separate area not directly related to CWM training and is located south of Test Area 1. Tamarind Bay was deemed suspect after stressed vegetation was observed in the area during an archeological investigation. Test Areas 2, 3, and 7 were not investigated because interviewees indicated that no CWM testing was conducted at the northern end of Water Island. The MTA investigation of all sites included surface inspection, magnetometer sweeps, and select surface soil sampling (MTA, 1994). 1.3.4.3. There were no suspect anomalies or surface debris identified at Tamarind Bay, Test Area 6, Test Area 8, the portion of Test Area 4 north of the road, and the portions of the Flamingo Bay Landfill Area outside the fence. The remainder of Test Area 4, Test Area 5, and the fenced Flamingo Bay Landfill Area were further characterized by completing 100% magnetometer surveys, selecting areas or anomalies for investigation, and collecting surface soil samples. Chemical agent (CA) or agent breakdown products (ABP) were not detected during field screening or laboratory analysis of any of the surface soil samples collected at the former Fort Segarra. 1.3.4.4. MTA submitted recommendations for intrusive investigation for consideration by the Anomaly Review Board (ARB) in September 1994. MTA recommended specific anomalies and representative "hot" areas in Test Area 5 for intrusive investigation, as well as 10% of the 20-foot by 20-foot grid squares dividing the southern portion of Test Area 4 and the fenced portion of the Flamingo Bay Landfill Area. MTA also recommended sampling of the circular depression in Test Area 4 and the suspected trench area in the Flamingo Bay Landfill Area. The ARB approved MTA's recommendations in January 1995, but no intrusive work was performed. 1.3.5. 1995 Environmental Assessment In 1995, the DOI, Bureau of Reclamation, conducted an environmental assessment (EA) of Water Island for the Office of Territorial and International Affairs. W912D Y-04-D-0005, DELIVERY ORDER 0013 4/22/2009 / FINAL investigation was conducted in order to fulfill a requirement for the Water Island title transfer to one or more entities. The final version included a finding of no significant impacts (FONSI), and concluded that an environmental impact statement was not required to implement the title transfer of Water Island from the DOI to former sub- lessees and the Virgin Islands Government (DOI, 1996). 1.3.6. 2001 Historical Photo Analysis In September 2001, the U.S. Army Corps of Engineers, Engineering Research and Development Center, Topographic Engineering Center (TEC), published a report on the historical photo analysis of Water Island (TEC, 2001). While the report covered all of Water Island, it focused on Test Area 4, Test Area 5, and the Flamingo Bay Landfill Area. 1.3.7. June 2001 Revised Archives Search Report The U.S. Army Corps of Engineers, St. Louis District, completed revising the ASR for the former Fort Segarra in June 2001. The revised ASR updated the 1991 ASR by Ebasco to include information made available after the previous report (USACE, 2002). 1.3.8. 2000-2003 Site Visit and Engineering Evaluation/Cost Analysis 1.3.8.1. In July 2000, Parsons conducted a site visit to Water Island to assess sites presented in the ASR. The visit focused on the Flamingo Bay Landfill Area, Test Area 4, and Test Area 5. Observations made during the site visit included: • Test Area 4 is covered with junk cars and trucks. • Test Area 5 is currently the site for solid waste staging on Water Island including dumpsters, household trash, and recycling containers. 1.3.8.2 Based on MTA's recommendations from the 1994 Site Characterization, Parsons was contracted to perform an Engineering Evaluation and Cost Analysis (EE/CA) at the former Ft. Segarra. To achieve the objectives of the EE/CA, Test Areas 4, 5, 8, and the Flamingo Bay Landfill required sufficient characterization concerning the presence of chemical warfare materiel and chemical agent contaminated media (See Figure 1.1). The characterization goals included conducting test pit and trenching investigations, collection of soil samples, interpretation of the data, and documentation of the findings. The characterization data were used to determine if chemical warfare materiel-contaminated areas exist at each site and to identify appropriate response alternatives. 1.3.8.3. No CWM was identified during the EE/CA; however, chemical agent contaminated media in the form of 3X scrap was identified during the investigation. Specifically, a vented one-ton container was identified on the landfill surface of the Flamingo Bay Landfill and what was believed to be a concrete-filled bomb casing was identified at the waterline near the landfill. The former contents of both are unknown; therefore, they were assumed to be 3X scrap. The EE/CA Report recommended that these 3X scrap items be removed from the site and properly incinerated in accordance DoD policies and regulations. Also included in the EE/CA Report is the recommendation 1-6 l:\HUNT-MRS PROGRAM\PROJECT.S\D013, SEGARRA\REPORT\FINAL\SEC-l_INTRO DOC REV: 0 CONTRACT NO. W9I2DY-O4-D-0OO5, DELIVERY ORDER 0013 4/2i<2009 FINAL for site-wide institutional controls. These institutional controls would consist of an informational sign at each of the two docks on Water Island with accompanying informational brochures. The EE/CA Action Memorandum was signed and the 3X scrap Removal Action and site-wide institutional controls for the former Ft. Segarra have been approved. 1.3.8.4. On March 29, 2005, the properties which included the areas of interest investigated during the EE/CA were transferred from the Department of the Interior to the government of the USVI. All of the areas of interest are undeveloped. Test Area 4, Test Area 5, and the Flamingo Bay Landfill Area are currently used for the storage of various types of debris and garbage. 1.4 PURPOSE AND SCOPE 1.4.1 The purpose of Delivery Order 0013 was to perform a RA to remove and dispose of the 3X scrap previously identified in the Flamingo Bay Area of Water Island. The 3X scrap requiring removal and disposal included an empty one ton container and what was believed to be a M78 bomb body filled with concrete. The scope included: • Preparation of a Work Plan (finalized May 2007); • Provide accurate dimensions of the scrap for proper identification; • Removal, disposal, and documentation of the removal and destruction of potential 3X scrap; and • Preparation of a Site Specific Final Report (this document) to summarize the activities of the RA. 1.4.2 A secondary objective of the scope was to obtain government acceptance of a design for two permanent signs required for institutional controls as well as installation of these signs. 1.5 PROJECT TEAM This subchapter describes the organizations along with their project role. An overall organizational chart for the RA is presented in Figure 1.3. Table 1.2 lists the organizations and their category of responsibility. In addition to the key organizations listed in Table 1.2, local governments and other stakeholders had roles in the project. CESAJ responsibilities included review of project plans and documents, obtaining rights-of-entry to properties in the work area, working with the news media and the public, and coordinating with federal, U.S. Territory, and local agencies on issues pertaining to implementation of this project and protection of ecological and cultural resources. Other responsibilities included providing proper notifications to the U.S. Environmental Protection Agency (USEPA) and U.S. Territory regulators regarding any transportation operations, notifying the National Response Center and the U.S. Territory officials in the event of a release or spill, and signing the hazardous waste manifest as generator of any hazardous waste. CESAJ provided an on-site representative to handle waste manifests and to coordinate public affairs. 1.5.2 U.S. Army Engineering and Support Center, Huntsville (USAESCH) USAESCH is the implementing agency for execution of this project, provided technical expertise for munitions and explosives of concern (MEC)/CWM activities, and served as the Project Manager for conduct of the RA. USAESCH responsibilities included procurement and direction of the prime contractor (Parsons) and supporting agencies, and the coordination of document reviews and approvals. As the Project Manager, USAESCH was responsible for directing the contractor and controlling the budget and schedule. 1.5.3 Parsons 1.5.3.1 Parsons, as the prime contractor to USAESCH, prepared a RA Work Plan, provided overall engineering support and services for implementation of the RA, and prepared this report detailing the work completed. Parsons was responsible for performance of the activities detailed in the PWS. 1.5.3.2 Parsons subcontracted some services to ensure successful completion of the delivery order. These services included, but are not limited to: 1. Site support (site preparation and access, temporary storage, and equipment transportation). 2. Transportation and disposal of waste streams. 3. Site restoration. 1.5.4 Other Stakeholders The various property owners and neighbors were contacted if it was determined that they would be impacted by the operations. 1.6 TECHNICAL APPROACH The approved Removal Action WP (Parsons, 2007) included the plans listed below as required by DID MR-005-01. 1-8 l:\HUNT-MRS PROGRAM\PROJECTSUX>13, SEGARRA\REPORT\FINAL\SEC-l_INTRO.DOC REV: 0 CONTRACT NO. Segarra was granted by the USAESCH on September 24, 2007. Operations to conduct the removal action at the former Ft. Segarra began with the arrival of Parsons' personnel on November 29, 2007. Preparation activities for the disposal were conducted November 29 through December 2, 2008. The removal action commenced on December 3, 2007 in accordance with the approved Final WP (Parsons, 2007). The 3X scrap was securely staged in a fenced area on Water Island until removal from the site and transfer to Veolia Environmental Services on December 16, 2008. 2.1.2 Two informational signs were designed and installed on Water Island to notify the public of the previous DoD activities on the island. The signs were installed at the public ferry dock and the deep water dock on Water Island in January 2009. 2.2 WORK PERFORMED 2.2.1 Preparation for the RA operations were conducted on November 29 and 30, 2007. During this time, a site visit was conducted and the 3X scrap was verified as having been secured within a fenced area just outside of the Flamingo Bay Landfill. On December 3, 2007, ProMar (subcontractor) began cutting the 3X scrap so pieces would fit into 30 gallon, burnable, DOT-approved polyethylene drums. 2.2.2 During the scrap processing, two measurements were obtained which would aid in the correct identification of both the bomb casing and the one-ton cylinder scrap. The bomb was recorded as having an 18.63 - 18.78-inch diameter, as well as a length of 53-inches. These measurements are consistent with the Bomb, Chemical, 1000-pound, AN-M79 - not the 500-pound M78 as formerly believed. The AN-M79 were recorded as being stored in the Toxic Storage Yard at the former Ft. Segarra, apparently for the purpose of determining the degradation of the agent CK in a tropical environment. Additionally, several attempts were made to determine the thickness of the one-ton container. After years of corrosion from the elements, an accurate measurement was not possible. The measurements obtained from the center of the cylinder were slightly less than 3/8 of an inch (0.375). This measurement should support the fact that the cylinder type was a Type A or D (0.406 inches) as opposed to a Type E thickness of 0.28125 inches. Types A and D 1-ton storage cylinders were traditionally used to contain any type of liquid or gaseous fillers. Historical records indicated that 1-ton cylinders at Fort Segarra were used to contain CG, H, HD, GA, NO2, and butane. 2-1. W912DY-04-D-0005, DELIVERY ORDER 0013 4/22/2009 FINAL 2.2.3 After all scrap was processed, the Site Safety and Health Officer (SSHO) certified the scrap as being free of explosives. This certification was recorded on a DD Form 1348-1A for each of the 26 drums. Additionally, the drums were labeled each with a unique identifier in accordance with the Work Plan. The 26 waste drums generated during the removal action were then relocated to a nearby fenced area (on Water Island) which was provided by the subcontractor. The drums remained secured in this location until waste certification and transportation could be coordinated. 2.2.4 On December 15, 2008, the Site Manager for this project mobilized again to Water Island to coordinate the pick-up of the 26 drums stored onsite. Each drum was inspected to make sure the contents remained unchanged since the last mobilization. Due to the weather conditions on the island, most of the plastic seals on the drums were corroded and unreadable. All seals were replaced with metallic seals and the new numbers were recorded in the drum log. A CESAJ representative was present onsite on December 16, 2008 to inspect the waste being transferred to Veolia Environmental Services. The 26 drums of scrap were loaded onto a barge and transported to the Tropical Shipping location on St. Thomas. Once at St. Thomas, the shipping documentation was properly executed and the waste was turned over to Veolia Environmental Services. Veolia transported the drums for waste treatment to their incinerator in Port Arthur, Texas. Destruction documentation is provided Appendix D of this report. 2.2.5 Parsons presented USAESCH with four sign designs for the informational signs that would be installed on Water Island. The signs were reviewed and updated based on comments and the final design was forwarded to local representatives for concurrence and approval. The final informational sign design selected for installation on Water Island is presented on Figure 2.1. 2.2.6 The construction of the sign was a key factor in maximizing the longevity of the sign. Due to the extreme weather conditions, exposure to intense sunlight, and corrosive nature of the Caribbean air, two signs and bases were constructed with the following characteristics: • Two, 36-inch by 24-inch flanged Vitratek® Porcelain Enamel panels for superior heat and ultraviolet resistance. The sign design was transposed on the panels using a four color printing process to ensure high-quality and vibrant image transfer. • Two Corten® steel cantilever frames for mounting the signs. The Corten® steel has an oxidized finish that will maximize the life of the sign and the low profile shape will minimize wind resistance. The signs were purchased from and fabricated by KVO Industries Incorporated in Santa Rosa, California. ProMar installed the signs in January 2009. 2-2 I:\HUNT-MRS PROGRAM\PROJECTS\DOn. W912DY-04-D-0005, DELIVERY ORDER 0013 4/22/2009 FINAL 2.3 FIELD PROCEDURES 2.3.1 Removal Action The removal action of 3X scrap was performed in accordance with the procedures outlined in the Site-Specific Work Plan. This action was performed by a small field team consisting of a Site Manager, SSHO, and a subcontractor with three employees. The Site Manager organized all the logistical matters in regards to the scrap removal from the site. The SSHO was in charge of overall site safety, provided unexploded ordnance (UXO) assistance if needed, and certified that the scrap was free of explosive content. The subcontractor's responsibilities were to cut the scrap so it could be contained in Department of Transportation (DOT)-approved shipping containers and assist in the transportation to the waste contractor. 2.3.2 Equipment The equipment used during the RA activities included: • Schonstedt Magnetic Locator (Model GA-52Cx); • Hydraulic punch and skid-steer loader; • Metal cutting equipment; and • Miscellaneous hand tools. 2.3.3 Scrap Management All scrap generated from the bomb casing and one-ton cylinder was certified as free of explosives by the SSHO. A DD Form 1348-1A was issued for all drums containing the scrap, and each drum was labeled and sealed to maintain integrity. Once processed, the drums were securely stored inside of a fenced holding area. 2.3.4 Sign Installation Areas The locations for the informational signs were selected in order to have the most exposure to visitors arriving on Water Island. Taking this into account, the signs were installed at the public ferry dock and the deep water dock on Water Island. These locations are the main access points for individuals arriving on Water Island. Figure 1.2 shows the locations where the informational signs were installed. 2.4 FACILITIES The RA at the former Ft. Segarra did not require the use of on-site facilities. All drums were also sealed and labeled with a unique identification number and securely stored in a fenced storage area until the waste transfer took place. 2.5.3 Sign Installation The two informational signs developed for Water Island were installed in accordance with the PWS (Appendix A) and according to the instructions provided by the sign manufacturer. The U.S. Government acquired Water Island in 1944 to serve as part of the coastal defense system for the U. S. Virgin Islands during World War II. The main fortification, Fort Segarra, located at the southern tip of the island, was constructed to protect the entrance to St. Thomas harbor. It consisted of two large gun emplacements, watchtowers, underground bunkers, and other military facilities. World War II ended before the two large guns could be installed. Other U.S. military installations existed on the main island of St. Thomas, including the U.S. Navy Submarine Base at Crown Bay and the U.S. Marine Corps Base at Bourne Field, now Cyril E. King Airport and the University of the Virgin Islands. From May 1948 to September 1950, the U.S. Army Chemical Corps occupied Water Island. Tests were conducted to evaluate various chemicals and their storage containers for use in tropical climates or jungle warfare missions. In 1952, the U.S. Department of Defense transferred Water Island to the U.S. Department of the Interior, The transfer of Water Island from the U.S. Department of the Interior to the U.S. Virgin Islands began in 1992, transfer was completed in 2005. Investigations by the U.S. Army Corps of Engineers concluded that all military munitions were removed from Water Island. Field operations and correspondence related to the removal action was documented and copies kept at the Parsons home office in Norcross, Georgia. 3.2 FIELD ACTIVITY REPORTS 3.2.1 Daily Status Reports 3.2.1.1 Daily Status Reports were written by the Parsons SM. These reports recorded, in summary form, the project progress and events that occurred daily. The Daily Status Reports are provided in Appendix B. 3.2.1.2 The Parsons Daily Status Reports documented the weather, personnel on- site, and daily events. Some of the items documented on the Daily Status Reports included: • Personnel on site • List of Equipment • Work planned for the day • Visitors to the site • Work accomplished • Work hours onsite 3.2.2 Photographic Records Photographs of the sign installation locations and site activities, packaging operations, and demobilization were taken during the removal action and are included in Appendix C. 3.3 DISPOSAL RECORDS 3.3.1 Waste generated during the removal action included metal and concrete debris. Information on the 26 drums of waste generated during the removal action is summarized in the Container Log contained in Appendix D. Each 30-gallon polyethylene drum contained approximately 150-pounds of metal debris and/or concrete generated from the demilitarization of the bomb and 1-ton cylinder. A DD Form 1348-1A certifying that the waste debris was free of explosives was completed for each drum. 3.3.2 The 26 drums of waste were shipped off-site on December 16, 2008. A copy of the Veolia waste manifest (manifest No. 000081299VES) is included in Appendix D. DELIVERY ORDER 0013 REV:0 4/22/2009 FINAL CHAPTER 6 SUMMARY AND CONCLUSION 6.1 REVIEW 6.1.0 Parsons was contracted by USAESCH to conduct a RA of potential 3X scrap at the former Ft. Segarra on Water Island, U.S. Virgin Islands. Work was conducted in accordance with the PWS and focused on the removal of munitions debris scrap from a site formerly known to provide CWM testing following WWII. 6.1.1 Items removed from the investigation site included a one-ton container and a concrete-filled AN-M79 bomb body. 6.2 LESSONS LEARNED Lessons learned during the RA are documented below to provide insight for future projects. The lessons learned include: • When working remote locations such as this one, constant communication and planning are a necessity to ensure all objectives are met. • Consider shipping most of the project equipment to the US VI because some items are not readily available or are hard to access in a timely manner. 6.3 CONCLUSION The RA at the former Ft. Segarra safely and effectively removed and disposed of all 3X scrap in accordance with the procedures prescribed in the approved WP. THOMAS, U.S.V.I. 1.0 OBJECTIVES The primary objective of this task order is for the Contractor to perform a removal action (RA) to remove, dispose and document removal of potential 3X scrap at the former Ft Segarra. The secondary objective to this task order i9 to obtain government acceptance of a design for a permanent sign required for institutional controls and Installation of this sign. 1.1 REGULATORY GUIDELINES The work required under this performance falls under the Defense Environmental Restoration Program/Formerly Used Defense Sites (DERP/FUDS). 1.11 The work associated with this Task Order shall be performed to conform to the requirements of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) Section 104, and the National Contingency Plan (NCP), Sections 300.120(d) and 300.400(e) as described in ER 200-3-1 and other U.S. Army Corps of Engineers (USACE) implementing guidance. 1.1.2 All work shall be conducted in full com pliance wi th Department of Defense (DoD), Department of Army, USACE, state and local requirements regarding personnel, equipment, and procedures. 29 CFR 1910.120 shall apply to all actions taken at this site. 1.1.3 Chemical Warfare Materiel (CWM) - The 2004 Phase II E E/CA investigation did not recover any Chemical warfare Materiel or any evidence that CWM remains at this location. Based on previous work at the site and the non-intrusive type work being performed in this task order, the probability of encountering buried CWM is deemed unlikely. Therefore, the areas under this Task Order are not suspected of containing CWM. However, during fieldwork, if the contractor identifies or suspects CWM, the contractor shall immediately withdraw upwind from the work area and contact the U.S. Army Engineering and Support Center (USAESCH) and the appropriate point of contact in their Work Plan (WP)/Accident Prevention Plan (APP). Additional reporting instructions are contained in CEMP-CE Memorandum, Notification Procedures for Discovery of W9I2DY-04-D-0005 0013 Page 5 of27 Recovered Chemical Warfare Materiel (RCWM) During USACE Projects, http://www.hnd.usace.arm v,mil/oew/Dolicv/IntGuidReQs/RCWM%20Notification%20memo w e ncl23%20April%2004.odf • 1.2 ROLE OF GOVERNMENT PROJECT PARTICIPANTS 1.2.1 U.S. Army Engineer District, Jacksonville District (CESAJ) - This office is the District Project Manager for work at the former Camp Sibert and is responsible for public affairs, real estate rights of entry, overall coordination of the Work, funding, regulatory interface, status of hazardous waste permits, and is the hazardous waste generator for this site. 1.2.2 U.S. Army Engineering and Support Center, Huntsville (USAESCH) - This office is responsible for executing this contract and addressing the potential 3X sc rap hazards at the site. 1.3 PUBLIC AFFAIRS 1.3.1 The Contractor shall not publicly disclose any data generated or reviewed under this contract. The Contractor shall refer all requests for information concerning site conditions to the local Jacksonville District Public Affairs Office, unless specifically authorized by the District with a copy furnished to the USAESCH PM. Reports and data generated under this contract are the property of the DoD and distribution to any other source by the Contractor, unless authorized by the Contracting Officer, is prohibited. 1.4 QUALITY CONTROL 1.4.1' Task Order Quality Management 1.4.1.1 The Contractor shall implement quality control processes as defined in a Quality Control Plan (QCP). The Contractor is responsible for ensuring that all work under the contract is of the quality that meets or exceeds contract requirements. The Government will implement quality assurance (QP) processes as defined in a quality assurance surveillance plan (QASP) to assure that contractor QC methods are effective and that a task order objectives and defined quality standards are met or exceeded. 1.4.2 Quality Control Plan (QCP) 1.4.2.1 The Contractor shall implement an acceptable Quality Control Plan(QCP). The QCP shall be detailed and comprehensive and shall cover all aspects of the task order activities impacting quality of deliverables and services. The Contractor's QCP shall be included in the W912DY-04-D-0005 0013 1.4.3 Quality Assurance 1.4.3.1 The Government will perform quality assurance (QA) of the Contractor's performance under this task order using the method of surveillance specified in the Quality Assurance Surveillance Plan (QASP). A sample QASP has been provided in Appendix A. The specific surveillance tasks performed under the surveillance plan will be defined following acceptance of the Contractor's QCP. The Government reserves the right to modify the surveillance tasks in the QASP at any time. The Government reserves the right to perform QA inspections at any time. QA failure can be defined as workmanship or work products not complying with the WP, PWS or not meeting project needs and/or objectives. Failure can also be defined as workmanship not complying with basic safety concepts and other industry safety practices. If any government QA review identifies a process failure or a work product failure, the Contractor will be issued a Corrective Action Report (CAR). The contractor shall provide full documentation detailing the root cause o f the fail ure, why it was not detected in the Contractor's QC Program, and how the problem was corrected. 1.4.4. Re-performance 1.4.4.1 Any service or submittal performed that does not meet task order requirements shall be corrected or re-performed be the Contractor and at no additional cost to the Gover nment. If the Contractor performs any task unsatisfactorily and all defects are not corrected, the Government reserves the right to terminate the PWS for defect. In addition, the Government reserves its rights under FAR clause 52.246-4, Inspection of Services - Fixed Price, for further remedies concerning a Contractor's failure to perform in conformance with contract requirements. 2.0 BACKGROUND 2.1 LOCATION 2.1.1 The former Fort Segarra is located on Water Island, fourth largest of the U.S. Virgin Islands, approximately 1,800 feet from St. Thomas Island and directly south of the capital, Charlotte Amalie, see Figure 1 Appendix B. 2.1.2 Water Island is approximately 500 acres, 1.75 miles long and 0.5 miles wide. Water Island has steep, rocky slopes and a highly indented coast with a maximum elevation of 290 feet. Access is primarily by boat via two docks, one small dock located on the west side of tije-is landaw^w,. and the other being the deep-water dock on the south side of F lamingo Bay. There is also a helicopter pad on the northern side of Flamingo Bay. In 2000, Water Island had a population of W912DY-04-D-0005 0013 161 residents, living in 80 households (U.S. Census Bureau, 2002). Currently, there are no commercial facilities on the island.. 2.2 SITE HISTORY AND USE 2.2.1 Fort Segarra was used for Chemical Warfare Materiel (CWM) testing from May 1948 to September 1950 and was transferred to the Department of the Interior (Dol) in 1952. An Engineering Evaluation and Cost Analysis (EE/CA) was conducted during Summer 2003 at Test Area 4, Test Area 5, Test Area 8, and the Flamingo Bay Landfill Area to characterize the nature and extent of chemical agent contamination, identify the presence of chemical agent contaminated media, identify potential safety concerns associated with recovered chemical warfare materiel (RCWM), study risk management alternatives, recommend response action alternatives, and document the recommended alternative. 2.2.2 The EE/CA conducted at the former Fort Segarra did not confirm the presence of RCWM. During the EE/CA, two suspect CWM-related items (a vented one ton container, see Figure 2, and a concrete-filled M78 500-lb bomb, see Figure 3 in Appendix B) were identified in the Flamingo Bay Landfill Area; however, there is extremely low risk to public safety and the environment from these 3X scrap items, since they have been demilitarized since the closure of the former Fort Segarra and have been significantly weathered since that time. These potential 3X scrap items are considered to be potential agent-contaminated media but soils sampled from inside and under the one ton container were non-detect for agent and agent breakdown products. The bomb body has been repeatedly submerged in seawater. A response action to mitigate potential remaining 3X scrap hazards and manage residual risk has been approved for Test Area 4, Test Area 5, Test Area 8, and the Flamingo Bay Landfill Area. 3.0 SPECIFIC TASKS Methods to be used to achieve task order objectives at the specified level of performance shall be determined by the Contractor. The Contractor will be evaluated periodically during each of the following tasks to ensure compliance with the PWS and to document that quality objectives, delivery schedule, and the overall completion date are being met. Failure to adequately complete any service or submittal to at least a satisfactory level of quality or timeliness may result in a repeat of the work, or a poor performance evaluation. 3.1 TASK 1 - OPTIONAL - FIRM FIXED-PRICE - PROJECT PREPARATION AND SITE v,s,T • - j^$liW^^iM^^^- The Contractor is authorized a post award site visit. Trie first objective of this task is for the Contractor to familiarize staff on the specifics of the site and prepare and plan strategies to W912DY-04-D-0005 0013 Page 8 of27 enable completion of the objective of this Delivery Order. The Contractor shall notify the USAESCH PM and prepare and submit an abbreviated Accident Prevention Plan for acceptance by the Governm ent 14 days prior to a planned site visit. The Contractor shall provide a trip report to the PM 7 days after conclusion of the site visit. In this report the contractor shall identify any applicable permits, local regulatory guidance, local building and design codes, the disposal facility and other constraints on this project. 3.2 TASK 2 - FIRM FIXED-PRICE -WORK PLAN (WP) The objective of this task is for the Contractor to submit and gain approval of a WP that is a detailed and comprehensive plan covering a]J aspects of the removal action IAW DID MR-005- 01 and MR 005-06: The WP shall have contingencies for the unlikely event that a CWM item is located during removal operations. The WP will be finalized at an on-board review after Draft comments have been incorporated. USAESCH will provide technical information on one ton containers and M78 Bombs to assist the Contractor in the planning process. 3.3 TASK 3-FIRM FIXED-PRICE-RESPONSE ACTION 3.3.1 The objective of this task is for the Contractor to safely remove and dispose of potential 3X scrap, a One Ton Container and an M78 bomb body filled with concrete, see Figures 2 & 3. The Contractor shall deliver the potential 3X scrap in a manner in which the disposal facility deems acceptable for processing in their incinerator. All work shall be IAW applicable Federal, State and local regulations. The Contractor shall provide the wall thickness of the one-ton container in the Site Specific Final Report. 3.3.2 Final Disposition Of MEC And Munitions Debris (MD) The Contractor shall furnish all necessary personnel and equipment to make final disposition on all recovered MEC/MD. The methodology to accomplish this task shall be proposed in the QCP and in accordance with Appendix C. 3.4 TASK4 - FIRM FIXED-PRICE -CONCEPTUAL DESIGN The objective of this task is for the Contractor to provide up to four different concepts for a permanent sign to include but not limited to different materials, styles, and types that are aesthetically pleasing, conveys the historic activities that took place at the facility, provides necessary precautionary statements, a point of contact (PCO) for findings, a PCO for more information with zero maintenance required. The Contractor shall include total cost, including installation cost, of each concept provided. Concepts will be provided in document form in Draft and Final versions. Concepts provided shall be able to meet applicable building and design W912DY-04-D-0005 0013 codes. The Final concepts will be presented to the local Government and Island Residents at which time a design will be selected. 3.5 TASK 5 - TIME & MATERIALS - DESIGN/INSTALL SIGN The objective of this task Is for the Contractor to submit and gain acceptance of a design package for two signs at two different locations on Water Island to include but not lim ited to sign design, structural design, installation instructions, manufacturer's warrantees, maintenance requirements, and meeting all applicable building codes and design codes for the selected sign design from Task 4. A Draft and Final Design Package shall be submitted. Once acceptance of the design package is obtained the contractor shall install the sign. The installation of the sign shall be considered complete upon passing Government inspection of sign after installation and delivery of as-built drawings. When a final sign design is selected in Task 4, Task 5 will be converted to a Firm Fixed-Price task. 3.6 TASK 6 - FIRM FIXED- PRICE • SITE SPECIFIC FINAL REPORT (SSFR) The objective of this task is for the Contractor to submit and gain approval of a SSFR in accordance with MR-030. The Contractor is to include a letter signed by an authorized person of the incineration company certifying that the potential 3X scrap has been incinerated and disposed. The Contractor is to also include a cover letter signed by an authorized person of the company certifying that the requirements of this Task Order have been met. The Contractor shall subm it a Draft and Final SSFR. 3.7 TASK 7 - FIRM FIXED-PRICE/UNIT COST - FIRM-FIXED PRICE/UNIT COST - PROJECT MANAGEMENT 3.7.1 The objective of this task is for the Contractor to perform project management activities necessary to maintain acceptable project control, to include but not limited to the following: 3.7.1.1 Schedule The Contractor shall submit a proposed Project Milestone/Payment Schedule in Primavera and Acrobat PDF for acceptance by US AESCH. The Contractor shall update the schedule in accordance with the basic contract. A final schedule shall be submitted a minimum of 30 days before commencing field work. Changes to the schedule must be accepted by US AESCH. 3.7.1.2 Telephone Conversations/Correspondence Records The Contractor shall keep a record of each phone convers ation and written correspondence concerning this Task Order in accordance with th§4jasJaeontract. A copy of this record rh?iiiJitfjHHi>tjt.. attached to the Project Status Report. / W9I2DY-04-D-0005 0013 3.7.1.3 Project Status Reports The Contractor shall prepare and submit Project Status Reports in accordance with the basic contract, DID MR-085 and including any other items required in the PWS. 3.7.1.4 Unit Cost The Contractor shall provide unit cost per month for not-in-the-field Project Management and unit cost per month for in-the-field Project Management. 3.7.1.5 Meetings The objective of this task is for the Contractor to provide a professional thoroughly fam iliar with the project, to attend a meeting at Huntsville, Alabama, and St Thomas Virgin Islands, as required. Meetings shall be assumed to be of one day duration. The Contractor shall be prepared to make presentations of proposed work, and summaries of work performed, as applicable, and shall entertain open discussions. In addition, the Contractor shall be prepared to show overheads or use other presentation techniques to convey plans, findings and recommendations, etc. The Contractor shall provide minutes and record of all meetings per the basic contract. For estimating purposes, the Contactor shall propose unit cost per meeting per site location, one in Huntsville and one in St. Thomas. 4.0 SUBMITTALS - GENERAL DIRECTIVES 4.1 COMPUTER FILES All final text fifes generated by the Contractor under this contract shall be furnished to the Contract Officer in Microsoft Word 2000 or higher software. Spreadsheets shall be in Microsoft EXCEL. Where applicable, all final CADD drawings shall be in Microstation 95 or higher. Each submittal shall contain a compact disk (CD) with electronic files of the submittal contained thereon. CDs shall be appropriately labeled as to its contents and date. 4.2 PDF DELIVERABLES In addition to the paper and digital copies of submittals, the final version of any and all reports and/or plans shall be submitted, uncompressed, on CD ROM in PDF format along with a linked table of contents, linked tables, linked photographs, linked graphs and linked figures, all of which shall be suitable for viewing on the Internet. The PDF files shall be created from source documents whenever poss ible. The U.S. Army Corps of Engineers (USACE) contractors executing projects will comply with the following procedures for processing MPPEH for final disposition. The objective of these procedures is to ensure that an inspection procedure of the exterior and interior surfaces of all recovered MPPEH is in place to ensure these items do not present an explosive hazard. These USACE contractor responsibilities and procedures will be contained, or referenced, in the project work plan. (1) Unexploded Ordnance (UXO) Sweep Personnel will only mark suspected items and will not be allowed to perform any assessment of a suspect item to determine its status. (2) Unexploded Ordnance (UXO) Tech I can tentatively identify a located item as MPPEH, followed by a required confirmation by a UXO Tech II or III (3) UXO Technician II will: (a) Perform a 100% inspection of each item as it is recovered and determine the following: Is the item a UXO, a DMM, munitions debris, or range related debris? Does the item contain explosives hazards or other dangerous fillers? Does the item require detonation? Does the item require demilitarization (demil) or venting to expose dangerous fillers? Does the item require draining of engine fluids, illuminating dials and other visible liquid hazardous, toxic or radiological waste (HTRW) materials? (b) Segregate items requiring demil or venting procedures from those items ready for certification. (c) Items found to contain explosives hazards or other dangerous fillers will be processed in accordance with applicable procedures. (5) UXO Quality Control (QC) Specialist will: (a) Conduct daily audits of the procedures used by UXO teams and individuals for processing MPPEH. (b) Perform and document random sampling (by pieces, volume or area) of all MPPEH collected from the various teams to ensure no items with explosive hazards, engine fluids, illuminating dials and other visible liquid HTRW materials are identified as munitions debris or range-related debris as required for completion of the Requisition and Turn-in Document, DD Form 1348-1 A. (6) UXO Site Safety Officer (UXOSO) will: (a) Ensure the specific procedures and responsibilities for processing MPPEH for certification as munitions debris or range-related debris specified in the work plan are being followed. (b) All procedures for processing MPPEH are being performed safely and consistent with applicable regulations. (7) Senior UXO Supervisor will: (a) Be responsible for ensuring work and Quality Control (QC) Plans specify the procedures and responsibilities for processing MPPEH for final disposition as munitions debris or range-related debris. (b) Ensure a Requisition and Turn-in Document, DD Form 1348-1A is completed for all munitions debris and range-related debris to be transferred for final disposition. (c) Perform random checks to satisfy that the munitions debris and range -related debris is free from explosive hazards necessary to complete the Form, DD 1348-1 A. (d) Certify all munitions debris and range-related debris as free of explosive hazards, engine fluids, illuminating dials and other visible liquid HTWR materials. A seal will bear the same unique identification number as the container or the container will be clearly marked with the seal's identification if different from the container. A documented description of the container will be provide by the contractor with the following information for each container; contents, weight of container; location where munitions or range-related debris was obtained; name of contractor, names of certifying and verifying individuals; unique container identification; and seal identification, if required. The contractor in a separate section of the final report will also provide these documents. 12-2. MPPEH Certification and Verification. a. The contractor will ensure that MPPEH is properly inspected in accordance with the procedures in I. above. Only personnel who are qualified UXO personnel will perform these inspections. The Senior UXO Supervisor will certify and the USACE OE Safety Specialist will verify that the debris is free of explosive hazards. b. DD form 1348-1A will be used as certification/verification documentation. All DD 1348-1A must clearly show the typed or printed names of the contractor's Senior UXO Supervisor and the USACE OE Safety Specialist, organization, signature, and contractor's home office and field office phone number(s) of the persons certifying and verifying the debris as free of explosive hazards. (1) Local directives and agreements may supplement these procedures. Coordination with the local concerns will identify any desired or requested supplementation to these procedures. (2) In addition to the data elements required and any locally agreed to directives, the DD 1348-1 A must clearly indicate the following for scrap metal: (a) Basic material content (Type of metal; e.g., steel or mixed) (b) Estimated weight (c) Unique identification of each of the containers and seals stated as being turned over. (d) Location where munitions debris or range-related debris was obtained. (e) Seal identification, if different from the unique identification of the sealed container. ) W912DY-04-D-0005 0013 (3) The following certification/verification will be entered on each DD1348-1A for turn over of Munitions debris or range-related debris and will be signed by the Senior UXO Supervisor and the USA CE OE Safety Specialist This statement will be used on any ranges where Range Related Debris is being processed along with munitions debris: "This certifies that the material listed has been 100 percent properly inspected and, to the best of our knowledge and belief, are free of explosive hazards, engine fluids, illuminating dials and other visible liquid HTWR materials. (4) The following certification/verification will be entered on each 1348-1A fir turn over of munitions debris and will be signed by the Senior UXO Supervisor on properties where only munitions debris is being processed: "This certifies and verifies that the material listed has been 100 percent inspected and to the best of our knowledge and belief, are inert and/or free of explosives or related materials." 12-3. Maintaining The Chain Of Custody And Final Disposition. a. The contractor, in coordination with the Corps of Engineers, will arrange for maintaining the chain of custody and final di sposition of the certified and verified materials. The certified and verified material will only be released to an organization that will: (1) Upon receiving the unopened labeled containers each with its unique identified and unbroken seal ensuring a continued chained of custody, and after reviewing and concurring with all the provided supporting documentation, sign for having received and agreeing with the provided documentation that the sealed containers contained no explosive hazards when received. This will be signed on company letterhead and stating that the contents of these sealed containers will not be sold, traded or otherwise given to another party until the contents have been smelted and are only identifiable by their basic content (2) Send notification and supporting documentation to the sealed container-generating contractor documenting the seal containers have been smelted and are now only identifiable by their basic content W912DY-04-D-0005 0013 (3) This document will be incorporated by the contractor into the final report as documentation for supporting the final disposition of munitions debris and range-related debris. (4) If the chain of custody is broken, the affected MPPEH must undergo a second 100 percent inspection, a second 100 percent re-inspection, and be documented to verify its explosives safety status (identified as either munitions debris or range related debris). b. Material that has been documented as safe in no longer considered MPPEH as long as the chain of custody remains intact. A legible copy of inspection, re-inspection, and documentation must accompany the material through final disposition and be maintained for a period of 3 years thereafter. 12-4. Material that is still MPPEH after inspection may be released only to a qualified receiver. The following must be accomplished prior to release of the property: a. Ensure that MPPEH that has been documented as hazardous is only transferred or released to those entities that: (1) Have the licenses and permits required to receive, manage, or process the materials. (2) Have technical experts about the known or suspected explosive hazards associated with the MPPEH. (3) Are qualified to receive, manage, and process MPPEH in accordance with DoD Instruction 4140.62. (4) Have personnel who are: (a) Experienced in the management and processing of hazardous materials equivalent to the MPPEH. (b) Trained and experienced in the identification and safe handling of used and unused military and/or any potential explosive hazards that may be associated with the specific MPPEH. b. The receiver must be advised of all of the potential hazards associated with the MPPEH and agree to receive and process the material IAW with DoD Instruction 4141.62. c. INTRODUCTION This Performance-Based Quality Assurance Surveillance Plan (QASP) has been developed pursuant to the requirements of the Performance-Based Statement of Work in Contract No. W912DY-04-D-0005 T.O. 0013. This plan sets forth procedures and guidelines that the USACE will use in evaluating the technical and safety performance of the Contractor. A copy of this plan will be furnished to the Contractor so that the Contractor will be aware of the methods that the Government will employ in evaluating performance on this contract and address any concerns that the Contractor may have prior to initiating work. 2. PURPOSE OF THE QASP • Confirm that the removal action is conducted utilizing proper procedures and in accordance with the approved work and safety plans; • Confirm that the Sign has been manufactured, designed and installed property • Define the roles and responsibilities of participating Government officials; • Define the types of work to be performed with required end results; • Document the evaluation methods that will be employed by the Government in assessing the Contractor's performance; • Provide the Surveillance Activity Checklist and Corrective Action Request (CAR) forms that will be used by the Government in documenting and evaluating the Contractor's performance; and • Describe the process of performance documentation. 3. METHODOLOGIES TO BE USED TO MONITOR THE CONTRACTOR'S V PERFORMANCE Even though the Government, through its COR, will be monitoring the contractor's performance on a continuing basis, the volume of tasks performed by the contractor makes technical inspections of every task and step impractical. Accordingly, USACE will use the Surveillance Activity Checklist (Attachment B.1) as the basis for monitoring the contractor's performance under this contract. The contractor's performance will be evaluated by the Contracting Officer using the performance metrics provided in Attachment B.2. PBSA In general, the work will be evaluated in terms of how well the requirements of the task order are satisfied, the extent to which the work performed follows the approach found in the contractor's technical proposal, clarity of documentation, and timeliness of scheduled task accomplishment. At the discretion of the COR or the Contracting Office or Specialist, other government officials approved by the Contracting Officer or Specialist may be asked to evaluate a particular deliverable or set of deliverables. 5. QUALITY ASSURANCE REPORTING FORMS The primary Form used to document surveillance activities will be the Daily Quality Assurance Report (QAR) provided in Attachment B.3. The OAR will be used by all team members to document surveillance activities conducted. All nonconformances will be documented on a Corrective Action Request (CAR), see Attachment B.4. Non- conformances are documented at the discretion of the person conducting the surveillance activity, but should be fair and reasonable. Each CAR will be annotated as a Critical nonconformance, Major nonconformance, or Minor nonconformance. CAR's will be provided to the Contracting Officer for distribution to the contractor. Completed forms by the contractor will be provided to the Contracting Officer by the date required per the nonconformance assigned; Critical, Major, or Minor. The contractor will be required to correct explosives/CWM safety issues immediately. All other CAR's. will provide a reasonable suspense date for the contractor to review and take appropriate action, usually 15 calendar days. These differentials are only applicable to work that has been specifically designated by the agency for ordance, explosives, and incendiary material differential pay. ** UNIFORM ALLOWANCE ** If employees are required to wear uniforms in the performance of this contract (either by the terms of the Government contract, by the employer, by the state or local law, etc.), the cost of furnishing such uniforms and maintaining (by laundering or dry cleaning) such uniforms is an expense that may not be borne by an employee where such cost reduces the hourly rate below that required by the wage determination. REQUEST FOR AUTHORIZATION OF ADDITIONAL CLASSIFICATION AND WAGE RATE {Standard Form 1444 (SF 1444) } Conformance Process: The contracting officer shall require that any class of service employee which is not listed herein and which is to be employed under the contract (i.e., the work to be performed is not performed by any classification listed in the wage determination), be classified by the contractor so as to provide a reasonable relationship (i.e., appropriate level of skill comparison) between such unlisted classifications and the classifications listed in the wage determination. Such conformed classes of employees shall be paid the monetary wages and furnished the fringe benefits as are determined. Such conforming process shall be initiated by the contractor prior to the performance of contract work by such unlisted class(es) of employees. The conformed classification, wage rate, and/or fringe benefits shall be retroactive to the commencement date of the contract. {See Section 4.6 (C) (vi)} When multiple wage determinations are included in a contract, a separate SF 1444 should be prepared for each wage determination to which a class (es) is to be conformed. The process for preparing a conformance request is as follows: 1) When preparing the bid, the contractor identifies the need for a conformed occupation) and computes a proposed rate). 2) After contract award, the contractor prepares a written report listing in order proposed classification title), a Federal grade equivalency (FGE) for each proposed classification), job description), and rationale for proposed wage rate), including information regarding the agreement or disagreement of the authorized representative of the employees involved, or where there is no authorized representative, the employees themselves. They are currently staged within a fence, just outside of the Flamingo Bay Landfill. • Inspected and photographed the cylinder, inert bomb, and drums. These photos will be uploaded to the Parsons server. The cylinder is extremely deteriorated and obtaining an accurate reading of the wall thicknesses may prove difficult (See photos). • Subcontractor stated that his employees would not be available to conduct the demilitarization until Monday morning at 0700. It is estimated that the demilitarization, packing, and documentation will only take 2-3 days. • Acquired transportation on Water Island. This will eliminate the need to walk from the ferry port to the Flamingo Bay Area each day. Work vehicles on St. Thomas and Water Island were equipped with first aid kits and fire extinguishers. • Mobilized back to St. Thomas to procure additional equipment for the upcoming operation. Once the Subcontractor and his employees arrived, Site Manager and SUXOS/SSHO conducted daily safety briefing, site-specific briefing, reviewed the work plan/APP/SSHP, and completed required training documentation. After reviewing measurements of the chemical bomb, the Site Manager performed some basic calculations and determined the bomb to have a diameter between 18.63 - 18.78 inches. These calculations, along with the bomb length of approximately 53 inches (listed in publications as 53.6 inches without fins), would make this a 1,000-lb bomb instead of a 500-lb bomb, as previously believed. According to publications, the 500-lb bomb had a diameter of 14.18 inches and a length (with fins) of 59.25 inches. There were no records on site showing the 500-lb bomb's length without fins. The correct nomenclature for a 1,000-lb is as follows: Bomb, Chemical, 1000-Pound, AN-M79. M79s where listed as stored in the Toxic Storage Yard of the former Ft. Segarra - Table 1.1 of the work plan. Apparently, the M-79s were used to determine degradation of the agent CK in a tropical environment. All bomb-related debris (bomb casing and concrete debris) was processed first. During the packaging, drums were filling up quickly based on the waste disposal contractor's original requirement of 100-lbs per drum (With each drum weighing approximately 20-lbs, this only leaves 80-lb of waste per drum). The Site Manager contacted Kim Elmore at Veolia and explained the possibility of filling all the drums and needing more drums to complete the operations. Ms. Elmore explained that the 100-lb limit was only a base-charge and that our price would increase based on a pcr-pound bases if we exceeded 100 pounds (including the weight of the drum). The Site Manager informed Ms. Elmore that we would fill each drum with 130 lbs of debris, and the total weight of the drum would be approximately 150 pounds in total. Ms. Elmore stated that Veolia would have no issue with that plan and we could proceed without ordering or waiting on additional drums. • The solid piece of concrete inside of the chemical bomb was broken up using a hydraulic-punch attached to a skid-steer loader. • Site personnel believe that the concrete-filled bomb was used as a mooring anchor. Based on the concrete-filled bomb's final resting location, it most likely washed up on shore with a boat attached to it. This is only an assumption, but pieces of metal rigging in the concrete seemed to support this conclusion. • Several attempts were made to determine the thickness of the one-ton container. After years of corrosion from the elements, an accurate measurement was not possible. The current measurements of the center of the cylinder were slightly less than 3/8 of an inch. This measurement should support the fact that the cylinder type is a Type A & D (0.406 inches) as opposed to a Type E (0.28125 inches). • Relocated all drums to an area supplied by the Subcontractor. New temporary fencing was constructed to secure the drums. • Placed drum seals on all drums and recorded each drum's unique drum ID. A total of 26 drums were filled during this operation and each drum weighed approximately 150 pounds. Because the large amount of metal flaking, corrosion, and small debris, it was easy to make each drum weigh approximately 150-lbs. • Secured drum storage area with available equipment. Additional measures to secure the area will be taken tomorrow. According to the Veolia Representative, a final assessment of the drum's weight will be made once they arrive in Port Arthur, Texas. • Cleaned area and returned location to original condition. • Photographed deep-water dock area for assessment of informative sign location. • Loaded truck with equipment and transported back to the public ferry dock. Unloaded equipment for Subcontractor so he could easily sort the equipment and incorporate needed gear back into his other operation as required (completing a fixed dock at the public ferry drop-off). 2.2 Operations Planned for Next Work Day: • Reinforce the fencing at the drum storage area. The container log was finalized and uploaded to the Parsons server. DD1348-1A forms were completed and signed. These forms will be scanned in and saved on the Parsons server once I'm back in the office. Contacted Mr. Dave Brewer at SHPO. Mr. Brewer stated that he has seen the final sign design and has no further comments. As far as Mr. Brewer is concerned, we should proceed to fabricate and install the sign. Mr. Brewer had additional questions about the remaining structures (pillbox, tunnels, etc.) on Water Island. He wanted to know if Parsons knew the status on the improvements of these structures by the Corps. I informed Mr. Brewer that the Jacksonville District would be better able to respond to those questions. Traveled to Home Depot and obtained some supplies to modify and improve the drum storage area fence. Mobilized to Water Island and drove to the Flamingo Bay Area. Used bailing wire to fabricate hinges for the temporary fencing door. Also used bailing wire to secure the fence closed. A "No Trespassing" sign was added to deter any curious visitors to the area. Traversed the island to collect more photo documentation. Took several photos of the pillbox area and tunnel entrance on the southern tip of Water Island. Mobilized back to the ferry dock and took several photos of the proposed sign location. Once finished, caught the ferry back to St. Thomas Island. Packed away equipment and site documents and prepared them for shipment back to the Parsons Norcross Office. This completes Phase I of the Ft. Segarra Response Action. Remaining to be completed for this task is drum shipment off Water Island and incineration at Veolia's incinerator. Also, obtain final approval of the informational sign design regarding Ft. Took ferry to Water Island and met with subcontractor (Marc Knopf, ProMar). Marc took me to Flamingo Bay for drum inspection and processing. Personally handled each drum to ensure they did not contain excessive amounts of water. As previously reported, some of the plastic drum seals had degraded. All plastic drum seals remaining (intact and otherwise) were removed and replaced with metallic drum seals. Documentation detailing former drum seal ID and new drum seal ID was completed. Made cell phone contact with the Corps Representative (Mr. Cruz) and Veolia Representative (Mr. Colon). Met Mr. Cruz at 1830. I showed Mr. Cruz where the Water Island ferry ported and gave him a tour of how to get there from his hotel. Mr. Cruz was also shown where to park to avoid being booted or towed during the daytime. Mr. Cruz and I will meet at 0600 in the morning to catch the ferry to Water Island. Could not make person to person contact with Mr. Colon. Mr. Colon believed we were staying in the same hotel and started feeling ill before he could travel to meet Emerald Beach to plan tomorrow's logistics. FIELD INFORMATION 2.1 Today's Operations: • Met Mr. Cruz (CESAJ Representative) at the Water Island ferry at 0600. Mr. Cruz and I mobilized to the Water Island and arrived at 0645. The subcontractor picked us up at 0715 and transported us to his house. • The trucks containing and the drums were driven from the subcontractor's house to the deepwater dock at Flamingo Bay and loaded onto the barge. Mr. Cruz and I rode on the barge to St. Thomas. • The barge ported at the Tropical Shipping location and we met with the Veolia Representative (Mr. Juan Colon). Mr. Colon was shown the waste and provided the shipping documentation for Mr. Cruz to review. • The drums were palletized and removed from the two trucks coming from Water Island. The drums were later transferred from the pallets to a Crowley conex. • Once the drums were loaded into the conex, they were secured using 2"x4"s and sheet metal screws. • The conex was locked and sealed with a shipping band. All shipping documentation was completed, reviewed, and signed for execution. • I transported Mr.Cruz back to Crown Bay Marina to his vehicle. Mr. Cruz left to make travel arrangements back to Puerto Rico. I caught the ferry back to Water Island and meet with the subcontractor to discuss the locations for the signage. I inspected each area with a Schonstedt for large anomalies. There appear to be no significant anomalies at the deepwater dock that are not associated with the dock foundation. There are no obvious anomalies at the sign location proposed for the public ferry dock. • Confirmed that the subcontractor will install the signs when his personnel return in January. Once installed, photos will be taken and emailed for verification. 2.2 Operations Planned for Next Work Day: • Field work has been completed for the removal action and final planning has been completed for sign installation.
