FIELD ACTIVITIES REPORT SITE NAME: L'HENRI, INC. (O1HENRY CLEANER) SITE LOCATION:HIGHWAY #38, #5B ANNA'S RETREAT, ST. THOMAS, USVI DATE: MARCH 6, 1995 TIME: 10:00 AM TO 6:00 PM WEATHER: Sunny, 85 F PURPOSE: Oversight of one soil boring at the O'Henry Cleaner. Caribbean HydroTech (driller) mobilize at from 10:00 to 1:00 pm IT Corp. (O'Henry Cleaner Consultant) mobilize from 10:00 to 1:00 pm; problems encountered: shipments did not arrive, had to go to Medical Supply store to get methanol. Air monitoring device was not present due to FedEx. Tailgate meeting held by IT Corp. at 1:15 pm. Present are Kenneth Loy, Greg Ward, Caroline Kwan, and the drilling crews (3 person). K.Loy briefed on hazards from drilling, contaminants found at this Facility, exposure route and symptoms when exposed enough of the contaminants. IT Corp. took 1 grab sample (ITSB01-0.5-0) at 5 feet for their field screening and 1 for CLP/VOA analyses at 14:20, clayey content. Auger refusal at 14.5 feet, tried 6 more inches in 20 minutes. IT informed the driller to stop and seal the bore hole. Bore hole was sealed from 4:10 to 5:15 pm, Drillers departed from Facility at 5:30 pm. Demobilize at left Facility at 6:00 PM. *64516* 64516 FIELD ACTIVITIES REPORT SITE NAME: L'Henri, Inc. (O1Henry Cleaner) SITE LOCATION: Highway #38, #5b Anna's Retreat, St. Thomas, USVI DATE:March 7, 1995 TIME: 8:00 to 3:00 PM WEATHER: Sunny, 85 F PURPOSE: Oversight Soil excavation at the O1Henry Cleaner Backhoe arrived at 8:45 AM, rest of the shipments arrived. Mobilize from 8:00 to 10:45 PM - stake out grids - lay out plastic sheeting for excavated soils - preparation of sample containers, labels - calibration of explosimeter and HNU meter - decon sampling equipments Tailgate meeting on the operation of backhoe held by IT (Greg Ward, operator)- briefed on hazards of backhoe when in operation. Start backhoe at 11:25 AM, Air monitoring was conducted throughout the excavation. No reading at any location except at one location around 2 feet deep less than 1 ppm. 10 soil samples were collected with 1 foot stainless steel auger 2 samples from each excavated location for field screening and CLP/VOA analyses. not enough sample was collected from each location to fill two 4 oz. jars, therefore, field screening analyses samples were collected as closely as possible to the CLP samples. - 2 to 3 feet, sample ID: EX01, at 12:35 PM - 2 to 3 feet, sample ID: EX02, at 13:00 - 2 to 3 feet, sample ID: EX03 at 13:20 (CLP) and 13:25 (Field Screening) - 1.7 to 2.7 feet, sample ID: EX04 at 13:40 (field Screening) and 13:45 (CLP) - 1.7 to 2.7 feet, Sample ID: EX05 at 14:05 (CLP) and 14:10 (Field Screening) IT informed me that one hit was detected at the 5 foot interval from the March 6, 1995 boring, 238 ppb, detection limit of the GC/MS was 5 ppb. (O'Henry Cleaner) SITE LOCATION: Highway #38, #5b Anna's Retreat, St. Thomas, USVI DATE:March 9, 1995 TIME: 8:00 to 7:00 PM WEATHER: Sunny, 85 F PURPOSE: Oversight Soil excavation at the 0'Henry Cleaner Tailgate meeting at 8:00 AM Ken from It approached me with the remediation of the excavated soil based on the two alternatives i gave him regarding the Land Ban Restriction requirements: treat on site-clean-up number is what EPA deems to be health based and if treated off site, the excavated soil has to treated to the BDAT (Best Demonstrated Available Technology) which is around 0.05 ppb based on the citation in the 40 CFR. Ken feels that if they drummed the waste and not treated, they are making a big mistake. He feels that passive venting of the excavated soil is appropriate at this facility. The approach he would take is to build layers of gravel and excavated soils which include pipe for gas venting. In few week, when they start monitoring the soil pile, if warranted, they would install an active blower. I concurred on his recommendation. At 10:00 Am, I called Carole Petersen, Branch Chief on the New York/Caribbean Superfund Branch II and informed her of this potential change of Scope pending to It speaking with the attorney this afternoon. Carole concurred. I informed Ken. Meanwhile Ken wrote up a variance to the approved workplan which detail It's recommendation to treating the excavated soil pile on site using passive venting. I signed and approved and informed him once built, the final report should included as much as detail on the construction of this passive venting system. Sampling of the excavated pit commenced from 10:35 Am to 17:06 PM. 13 samples were collected in the soil pile for confirmation purposes. Ken showed me the rest of the sampling results from 3/9/95 sampling. Depth is ok, lateral is ok still a little problem at the slab and by Liquor Barn (around the 200 ppbs values) 9;15 AM : begin back filling the pit with Devcon's sand and gravel. 14:00 to 16:30 samples packaging for preparation of shipping to labs. samples shipped at Airport FedEx office at 16:30. Ken informed them the wall needs to be at least six foot high based on his estimation of the size of the excavated soil. Worker informed Ken they need more cement blocks. Ken went to buy more materials. Nancy stopped by to check at 13:00. I spoke with her regarding to the work so far and the rest of the work to complete the soil ventilation. (O1Henry Cleaner) SITE LOCATION: Highway #38, #5b Anna's Retreat, St. Thomas, USVI DATE:March 12, 1995 TIME: 8:00 to 15:30 PM WEATHER: Sunny, 85 F PURPOSE: Oversight Soil excavation at the O1Henry Cleaner Mobilize at 8:00AM; Preparation of the passive venting system to be staged at the concrete slab at the O1Henry property. (O1Henry Cleaner) SITE LOCATION: Highway #38, #5b Anna's Retreat, St. Thomas, USVI DATE:March 13, 1995 TIME: 8:00 to 19:00 PM WEATHER: Sunny, 85 F PURPOSE: Oversight Soil excavation at the 0'Henry Cleaner Mobilized at 8:00 AM, Piping arrived 10:30 AM. Start laying out the piping for the gas venting layers, preparation of the gas venting layer, one foot of gravel, two feet of contaminated soil, another foot of gravel and another two feet of soil, there are PVC corrugated/slotted 2 inches diameter pipes across the bottom and at the mid-layer. Hard PVC pipes were used for vertical positions. Demobilized at 19:00. FIELD ACTIVITIES REPORT SITE NAME: L'Henri, Inc. (O'Henry Cleaner) SITE LOCATION: Highway #38, #5b Anna's Retreat, St. Thomas, USVI DATE:March 14, 1995 TIME: 8:00 to 12:00 PM WEATHER: Sunny, 85 F PURPOSE: Oversight Soil excavation at the O'Henry Cleaner Continuation of preparation of gas venting layers. Spoke with Henry Magras. He will have his local construction contractor build a roof and install roof turbines at the structure. I spoke him there someone will have to have access for future sampling and monitoring of the soil pile. Therefore, I told him to take that into consideration when his contractor build the roof and the entrance. Completed construction at 12:00 PM. FIELD INSPECTION REPORT SITE NAME: DEPT. OF EDUCATION (LAGA) SITE LOCATION: HIGHWAY #382, ANNA'S RETREAT, ST. THOMAS, USVI DATE: MARCH 13, 1995 TIME: 16:00 TO 16:30 PURPOSE: TO INSPECT THE PREVIOUS SAMPLED LOCATIONS WITH MS. JULIEMAE MONSANTOS, ENVIRONMENTAL SPECIALIST . I showed Ms. Monsantos the previous sampled locations and informed her the results of these samples. I informed her EPA's concern regarding to the high concentration of PCE found at the Plumbing room. She tried contacting the Superintendent of the School, but he was off-Island. She called Attorney Henry Thomas, Attorney General of the Virgin Islands. I spoke with him about EPA's concerns and he informed me he would like to see the location. We scheduled a site inspection for March 14, 1995 at 9:00 AM at the back of the Dept. of Education building. FIELD INSPECTION REPORT SITE NAME: DEPT. OF EDUCATION (LAGA) SITE LOCATION: HIGHWAY #382, ANNA'S RETREAT, ST. THOMAS, USVI DATE: MARCH 14, 1995 TIME: 9:00am TO 10:00am PURPOSE: To meet with Attorney Henry Thomas of the Virgin Islands Attorney General and to inspect the suspected source of PCE contamination. I met with Henry Thomas, Esq. at 9:00 AM. I showed him the area of concern (plumbing house). We also went in the back which is a big warehouse facility for the Dept. of Education to store their school material. We traced where the three underground pipes to the back of this warehouse. There seems to be a connection between these pipes to the back of this warehouse. There were a indentation on the floor that is directly in-lined with the three pipes in the plumbing room at the back of the warehouse. Attorney Thomas spoke with the building Supervisor and informed him to do not disturb these pipes and that EPA wants to know what is underneath the concrete where the pipes are, therefore, when EPA comes back to the Islands in April, Dept. of Education would probably have to use a concrete saw and cut open a 3 feet diameter around the pipes and take the slab out the see what is it down there. Meanwhile Attorney Thomas will go back his office and see if he has any plans and drawings of the building when it was first built or if any renovation was done to it.
