OPCMR AMENDMENT No. _2 OF PROFESSIONAL SERVICES CONTRACT THIS AMENDMENT made as of the 29th day of March , 2022 in the Territory of the Virgin Islands, by and between the Government of the Virgin Islands DEPARTMENT OF PROPERTY AND PROCUREMENT, on behalf of the DEPARTMENT of Health (hereinafter referred to as “Government”) and Pafford Medical Services, Inc. d/b/a Pafford Medical Services whose address is 3509 W 16" Street Hope, AR 71801 (hereinafter referred to as “Contractor”). OPCMR AMENDMENT No. _2 OF PROFESSIONAL SERVICES CONTRACT THIS AMENDMENT made as of the 29th day of March , 2022 in the Territory of the Virgin Islands, by and between the Government of the Virgin Islands DEPARTMENT OF PROPERTY AND PROCUREMENT, on behalf of the DEPARTMENT of Health (hereinafter referred to as “Government”) and Pafford Medical Services, Inc. d/b/a Pafford Medical Services whose address is 3509 W 16" Street Hope, AR 71801 (hereinafter referred to as “Contractor”). Both parties witness to being duly authorized to execute this agreement and will attest accordingly when and where necessary. WITNESSETH WHEREAS, the Government contracted with Contractor to provide assistance with the COVID-19 Mass Vaccination Program; and WHEREAS, pursuant to original Contract No. P125DOHT21 approved by the Governor on August 27. 2021, and Amendment No. | through Contract No. P143DOHT21, approved by the Governor on September 30, 2021 (which constitute and are hereinafter referred to as the “Contract”), the Government contracted with Contractor to provide assistance with the COVID- 19 Mass Vaccination Program: and WHEREAS. the Contract was renewed by renewal Contract No. P145DOHT21 which expires March 31, 2022; and WHEREAS, the parties desire to amend the Contract No. P145DOHT21 to extend its term by three (3) months, and to increase the compensation by Four Million Three Hundred Twenty Thousand Dollars and No Cents ($4,320,000.00); and NOW THEREFORE, for mutual and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties hereto agree as follows: TERMS AND CONDITIONS I. Paragraph 2 - The Contract No. P145DOHT21, is hereby amended and its term extended by three (3) months by deleting all existing language contained in Paragraph 2 and replacing it with language as follows: This term of this Contract shall be from October 1, 2021 to June 30, 2022. Upon the execution of the Governor this contract shall become effective for the term herein.
