Any false claims made in accordance with this application is grounds for penalties and fines being brought against the applicant. PLEASE READ THE INFORMATION ON THE BACK OF THIS BMV FORM NO: (2025-04) INSTRUCTIONS AND DOCUMENTATION REQUIRED This application must be completed before any action can be taken by the property owner; this allows the Bureau of Motor Vehicles (BMV) to make an attempt to contact the vehicle owner and notify them that an application has been filed to transfer the vehicle title. The BMV will notify the applicant by email or telephone and inform them when they may proceed. Upon Notice to Proceed from the BMV, Submit the Following Documents: • Copy of Property Deed or Property Tax Document (must clearly show owner's name) • Photos of the vehicle clearly shown on the property • Photos of the VIN from the dashboard and the driver's door • Photos of the license plate and stickers (if available) • Completed Affidavit for Title and Registration • Proof of Service by Publication (Published in two different newspapers for four consecutive weeks) • Proof of Service by Mail (Certified mail to last known owner and lienholder, if there is a lienholder) • Completed Title Application Form (Available at https://bmv.vi.gov under the "FORMS" tab) • Valid Government-Issued ID of the Property Owner NOTE: This application must be submitted by the property owner.
