MINNESOTA AIDS PROJECT HIV CASE MANAGER (posted 7/9/04) The Minnesota AIDS Project (MAP) is seeking a full-time Case Manager to work both in the Twin Cities and St. Cloud. The position is responsible for comprehensive client need assessments, risk reduction education, client advocacy, HIV disease and medical care education, identification of mental/chemical health issues and other client issues including end of life needs. Travel required for home visits in both locations. This position requires building successful working relationships with clients, medical and other social service providers. DESIRED QUALIFICATIONS: • An undergraduate or graduate degree in social work, public health, human services • At least two years experience providing social services in a job that entailed carrying a case load and performing the following duties: psychosocial assessments, interdisciplinary care coordination, resource development, development and utilization of client-centered care plans • Excellent verbal and written communication, problem solving and organizational skills. …
