The owner(s) of the vehicle or the agent authorized by the owner(s) is/are required to make the application for exportation of the vehicle. WHEN SHOULD THIS FORM BE COMPLETED? • When a vehicle that is used in the USVI is being exported out of the territory. • If the vehicle is not titled in the VI, the vehicle must first be titled then this form can be used. • If there were special arrangements made so that the vehicle did not have to be titled in the VI, please see a supervisor. REQUIRED DOCUMENTS • LETTER/DOCUMENT FROM LIENHOLDER AUTHORIZING THE SHIPMENT OF THE VEHICLE • ORIGINAL TITLE • VEHICLE REGISTRATION • OWNER’S ID • COMPLETED EXPORT DECLARATION VIN AND ODOMETER VERIFICATION FORM • IF SOMEONE OTHER THAN THE OWNER IS REQUESTING THE VERIFICATION, THE OWNER MUST GIVE POWER OF ATTORNEY (POA) TO AN AUTHORIZED REPRESENTATIVE. THE POA MUST BE NOTARIZED AND ACCOMPANIED BY THE OWNER AND AUTHORIZED REPRESENTATIVE’S IDS. PART A-1 MUST BE COMPLETED BY THE OWNER OF THE VEHICLE PART A-2 MUST BE VERIFIED BY THE INSPECTOR PART B MUST BE COMPLETED BY THE BMV STAFF PART C MUST BE COMPLETED BY U.S. CUSTOMS VERY IMPORTANT: IF THE VEHICLE WAS TRANSPORTED OUT OF THE TERRITORY WITHOUT THE REQUIRED DOCUMENTATION FROM THE BMV, THE BMV CANNOT VERIFY THE ODOMETER WITHOUT THE VEHICLE BEING PRESENT AT THE BMV.
