This form should be completed by the Authorized Representative of the business applying for a rental vehicle quota. 2. Quota Request. The Total Quota refers to the number of rental vehicles your business is requesting. If you are requesting additional vehicles beyond your current quota, fill out the Additional Quota section. 3. Required Documents (see below). Make sure all the necessary documents are included with your application to avoid delays. 4. Application Fee. An application fee is required. 5. Submission. Submit your completed form and documents to the nearest BMV office.
