AMENDMENT OF PROFESSIONAL SERVICES CONTRACT — THIS AGREEMENT made as of the 9 day of tae , 2017 in the Territory of the Virgin Islands, by and between the GOVERNMENT of the Virgin Islands DEPARTMENT OF PROPERTY AND PROCUREMENT, on behalf of the DEPARTMENT OF HEALTH, DIVISION OF CHRONIC DISEASE AND PREVENTION, (hereinafter referred to as “GOVERNMENT”) and CLEARWATER RESEARCH, INC. (hereinafter referred to as “CONTRACTOR’). Both parties witness to being duly authorized to execute this agreement and will attest accordingly when and where necessary. WITNESSETH WHEREAS, pursuant to Contract No. P022DOHC17 approved by the Governor on March 16, 2017, which constitutes and is hereinafter referred to as the “Contract”), the GOVERNMENT contracted with CONTRACTOR to provide services conducting a health survey; and WHEREAS, the parties desire to amend the Contract to change the termination date to March 31, 2018; and WHEREAS, the parties desire to amend the Contract to increase compensation for the BRFSS (Behavioral Risk Factor Surveillance System) main survey by SIXTEEN THOUSAND DOLLARS AND ZERO CENTS ($16,000.00) to a total of FORTY-EIGHT THOUSAND DOLLARS AND ZERO CENTS ($48,000.00); and WHEREAS, the parties desire to add the Zika Call-Back Survey beginning in April 2017; and WHEREAS, the parties desire to amend the Contract to set compensation for the Zika Call- Back Survey to a total of FORTY-FIVE THOUSAND DOLLARS AND ZERO CENTS ($45,000.00); and WHEREAS, the amendment of the Contract pursuant to this Amendment of Professional Services Contract shall not result in any change to the scope of services provided for under the Contract other than the aforementioned change in the addition of the Zika Call Back Survey (Amended Addendum I).
