FORM L&WD-6: 1) Submit current Tax Clearance Letter from VI Bureau of Internal Revenue for Liberty Mobile USVI, Inc. Additional Documents 1) Submit a cross-section profile of the area showing how excavation will affect topography. 2) Submit a list of plant species found at the site or provide any evidence of attempts to identify species of interest. 3) Submit a graphic depicting the coverage area of the tower and any study that provided the basis of the decision for the location of the tower.
